Summary
The Patient Safety Officer is a critical role in the Quality Management System to ensure safe patient care in the healthcare system. This critical role identifies, investigates, analyzes, reports, and follows up on safety events that affect or can affect patient safety throughout Confluence Health. The Patient Safety Officer helps drive the development, implementation, and maintenance of patient safety programs to maintain the highest standards of care. They have primary oversight of the facility-wide patient safety program. This role serves as a vital resource within the facility towards process improvements that support the reduction of medical care errors and patient harm. The Patient Safety Officer provides expertise for safety assessments, coordinates the activities of the patient safety committee, educates Confluence Health on system-based causes for medical errors, consults with the management and staff, and communicates proven best practices regarding effective safety strategies to others within the organization. This role directly impacts patient outcomes, and the quality of care provided by our organization.
Position Reports To: Director of Care Excellence
Essential Functions
- Work with all service lines to implement standard work around patient safety for reliable delivery of healthcare systems that incorporate best practice safety methodologies by promoting a Just Culture.
- Conduct Root Cause Analysis and Action and follow up to ensure event closure including reporting to Washington Department of Health if necessary
- Daily identification of patient safety events or near events in RL Datix with maintenance of spreadsheets to track and trend
- Preparation of cases for regular cadence safety meeting with acquisition of additional information from relevant departments/care providers
- Review of new events to look for prior trends and developing performance improvement activities and/or projects in collaboration with internal and external stakeholders
- Enter new RL Datix events as needed
- Manages the ongoing, organization-wide system of patient safety information collection, evaluation and remediation to identify deficiencies and opportunities for improvement according to Confluence Health standards and accrediting/regulatory requirements
- Identification of near miss ("good catch") with appreciation memos as appropriate. Will oversee the Good Catch program.
- Maintains continuous accreditation and regulatory readiness through compliance with patient safety standards from accrediting/regulatory agencies that review Confluence Health
- Actively participates in safety management and planning of the Confluence Health Safety Committee
- Documents, trends and plans for remediation of situations that present risk to patient safety
- Evaluate all incident reports as appropriate using the Confluence Health Reporting system and conduct trend analysis
- Attends required meetings and serves on committees as requested
- Training of team members as needed
- Maintains professional affiliations, attend as approved outside conferences, seminars, training, etc.
- Promote a culture of safety within the organization
- Other duties as assigned

Qualifications
Required:
- Current RN licensure in the state of Washington (RCW 18.88) or licensure through Multistate Nurse Licensure Compact (SSB 5499)
- 3 years in nursing experience
- Experience in medical record analysis and past participation in quality improvement projects.
Desired:
- Certification in healthcare quality preferred
- Certification in Patient Safety within two years of position by an accredited organization such as IHI or NPSF
- Five years of related experience in safety and regulatory compliance preferred
- Experience with RLDatix reporting system and EPIC
