Friday to Sunday

8:00 AM to 8:00 PM (Full-time) or 8:00 AM to 5:00 PM (Part-time)

*JOB DESCRIPTION SUMMARY*

The *Director of Clinical Services Assistant (DCSA) *assists in the on-going support of field clinicians and patients under the direct supervision of the Director of Clinical Services. The Director of Clinical Services Assistant builds from the resources of the community to plan and direct services to meet the needs of individual and families within their homes and communities.

*ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES*

Patient Care

1. Evaluates patient’s chart.

2. Assists with monitoring and accuracy of a patient centered plan of care. Plan of care includes:

A. Understanding of hospice, illness, and prognosis

B. Goals of care/advance care planning/proxy decision-making

C. Detailed symptom history

D. Psychosocial and coping history including loss history

E. Spiritual history

F. Functional assessment

G. Chart monitoring for accuracy and appropriate response when errors are discovered

H. Check daily charting for note completion and note signing

3. Schedule weekly CNA visits with DCS/RN Case Manager oversight.

4. Observes, records and reports to DCS or RN Case Manager any signs and symptoms which indicate a change in the patient’s condition.

Communication

1. Answers telephone and calls staff as appropriate.

2. Orders DME based on patient condition and RN request.

3. Assists the team members in coordination of care plan with interdisciplinary team members.

4. Calls all outside physicians listed on Hospice assignment (upon admission and discharge) and notifies DME company and Pharmacy.

Additional Duties

1. Ensures that all telephone orders are signed by the physician and returned to the hospice in a timely manner per organization policy.

2. Refers, tracks, and reports ordering and cost of DMS, medical supplies and medications related to the terminal illness to the DCS and/or Executive Director.

3. Completes incident reports as needed.

4. Supervises ancillary personnel and delegates responsibilities when required.

5. Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and in-service classes.

6. Actively participates in quality assessment performance improvement teams and activities.

7. Provides documentation and record keeping in accordance with professional standards and organizational policies.

*POSITION QUALIFICATIONS*

1. Graduate of an accredited school of nursing. One (1) to two (2) years of recent acute care experience in an institutional setting, and one (1) to two (2) years of recent experience in home care.

2. Current licensure in State and CPR certification.

3. Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.

4. Bachelor’s degree, with one (1) year of hospice care experience preferred.

5. Management experience not required. May be responsible for supervising hospice aides.

6. Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist.

7. Prolonged or considerable walking or standing. Able to lift, position and/or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping bending, kneeling and/or crouching. Visual acuity and hearing to perform required nursing skills.
